Job Overview

The care advisor profession in Australia is broad and diverse but consistent with all practice is a commitment to human rights and social justice. Social workers work with individuals, families, groups, and communities in the context of their physical, social and cultural environments, their past and current experiences, and their cultural and belief systems.

The NDIS/HCP Care Advisor will work collaboratively with participants by co-designing a goal oriented support plan, in this full time role. This role provides information, support, advice, advocacy and coordination services ensuring that the design and implementation of service options enhance customer’s independence, control, wellbeing and quality of life in accordance with Consumer Directed Care principles.

About You

Responsibilities and Duties:

  • Assisting people with disabilities and aged care packages to remain living at home for as long as possible
  • Support new HCP consumers and NDIS participants to connect to appropriate individualised services
  • Conduct assessments, develop support plans, undertake review, implement and facilitate clinical and care interventions
  • Liaise effectively and positively with external stakeholders including carers, acute, respite and other community services, medical and allied health professionals
  • Provide robust budget management, ensuring that accurate budgets are maintained so the customer can be supported as required
  • Demonstrated understanding of needs and issues of people with a disability from CALD backgrounds and their carers
  • Strong track record on project management and/or establishing new services
  • Strong experience in community engagement and relationship building
Qualification and Experience:
  • Tertiary qualifications in Community Health or a related discipline
  • Previous management of HCP consumers, NDIS participants or NDIS funds management
  • Must have disability experience working in a similar advisor/case management role
  • Knowledge of the HCP, NDIS and disability sector
  • Participants funds/budget management
  • Excellent interpersonal, communication, and problem-solving skills
  • A good understanding of work health & safety in home and community settings
  • Comprehensive understanding of the Aged Care Act and NDIS and Disability Act
  • Ability to self-motivate and take initiative
  • Well-developed interpersonal, negotiation and complaints resolution skills
  • Strong networking and relationship management skills
  • Current Police Clearance check
  • Current Working with Children Check
  • Current Driver’s License and comprehensively insured and registered vehicle
