Job Overview

The case manager profession in Australia is broad and diverse but consistent with all practice is a commitment to human rights and social justice. Social workers work with individuals, families, groups, and communities in the context of their physical, social and cultural environments, their past and current experiences, and their cultural and belief systems.

In all contexts, Case managers maintain a dual focus on both assisting with and improving human wellbeing and identifying and addressing any external issues (known as system or structural issues) that may impact on wellbeing or may create inequality, injustice, and discrimination.

Case managers may undertake roles in casework, counseling, advocacy, community engagement and development and social action to address issues at both the personal and social levels. Case managers also work in areas such as policy development, education and research particularly around issues of social justice, disadvantage, and the marginalisation of people in their communities or in society.

About You

The key to success in this role is the demonstrated ability in the below skills and experience:
Case Management experience with Home Care Packages / National Disability Insurance Scheme with experience in developing and managing a budget for aged or disability care Experience.
Ability to work autonomously and in a team environment Excellent time management & problem solving and communication skills with the ability to network.
Excellent communication and interpersonal skills to build rapport with various stakeholders and leadership

Responsibilities and Duties:

• Client meetings and assessment, referral, review and goal planning
• Client funds management and developing budgets for clients Relationship management with internal and external stakeholders
• Co-ordinate and manage services to our NDIS and Home Care Packaged clients
• Drive business development opportunities
• Promote AusCare in the community to potential clients
• Monitoring their progress and leading them
• Provide supervision and support to work placement candidates

Qualification and Experience:

☑ Tertiary qualifications in Social Work, Aged Care, Disability Services, Community Services Coordination, Frontline Management or equivalent
☑ A minimum of 3 years’ experience in NDIS or Aged Care Industry.
☑ Demonstrated ability with seeking opportunities to sign up new NDIS participants to grow the business
☑ Police Check and Working with Children Checks.
☑ Current First Aid Certificate (Desirable)
